245D Services (Home and Community-Based Services)
245D services are designed for people who need more structured, long-term support under a waiver program.

PCA (Personal Care Assistance) & CFSS (Community First Services and Supports)
PCA and CFSS services are designed to help individuals with day-to-day personal care tasks so they can live more independently at home and in the community.
Our services include, but are not limited to:

Homemaker services
Assist with routine household tasks—such as light housekeeping, meal preparation, shopping for essentials, and household organization—to help individuals maintain a safe, clean, and healthy home environment.

PCA and CFSS
Provides support in various areas, including activities of daily living (such as eating, bathing, and grooming), health-related tasks, and instrumental activities of daily living (like shopping, cooking, and laundry).

Individualized Home Supports (IHS)
Customized care to provide support and/or training in community living service categories, with three service options: IHS without training, IHS with training, and IHS with family training.

24-Hour Emergency Assistance
Caregivers provide immediate response, offering on-call counseling, problem-solving, and in-person support for personal emergencies at home that are non-injury related and not life-threatening.

In-Home Respite Care Services
Temporary relief for primary caregivers by offering short-term care and support to individuals in their own home. This service ensures the individual's needs are met while giving caregivers time to rest, manage personal responsibilities, or take a break. Support may include supervision, personal care, companionship, and assistance with daily activities.

Night Supervision Services
An overnight service provided to individuals who need support and monitoring while they sleep to ensure their health, safety, and well-being. Available under Minnesota’s 245D waiver services, this care is provided in the person’s own home by awake staff and may include assistance with repositioning, mobility, behavioral redirection, or other routine needs that arise during the night.

Individual Community Living Supports (ICLS)
Services are primarily designed for individuals aged 65 and older, including those on the Elderly Waiver or Alternative Care Waiver, and cover six core areas:
cognitive support, adaptive assistance, daily living activities, household management, health and safety, and community engagement.
